Position Profile
knownwell is seeking a compassionate and structured Patient Experience Coordinator to join its remote patient services team. Acting as the primary liaison for an innovative, nationwide weight management and primary care network, this professional will manage high-volume patient intake pipelines, execute complex clinical scheduling grids, and coordinate proactive care outreach across specialized digital health platforms.
Main Duties

Manage high-volume inbound and outbound patient communications, delivering empathetic front-line assistance regarding telehealth options and primary care programs.

Coordinate, track, and update patient appointment calendars across various nutritional, behavioral, and medical specialties.

Conduct proactive patient outreach to fulfill provider orders, close care loops, manage incoming medical referrals, and secure required follow-up care.

Record comprehensive operational data, patient inquiries, and scheduling updates inside Electronic Health Record (EHR) frameworks.

Facilitate new patient digital onboarding pathways, helping clients establish access to core applications and preparing them for initial provider visits.

Monitor operational service queues to resolve outstanding client tickets within established internal timelines, escalating urgent clinical inquiries to the appropriate care teams.
Ideal Candidate Profile

Education: High school diploma or equivalent qualification is required.

Domain Background: A solid history of five or more years navigating medical appointment scheduling, centralized patient access points, or medical call center rooms.

Technical Frameworks: Hands-on proficiency utilizing enterprise Electronic Health Record (EHR) systems, with direct experience using Athena highly preferred.

Core Aptitudes: Dynamic problem-solving capabilities, rapid data entry speeds under pressure, and the capacity to balance multi-system computer interfaces while speaking on the phone.
